For youth trips such as camps and missions, we require a current Medical Consent Form to complete the registration process. Medical Consent Forms should be updated yearly, or whenever there is a change in your teenagers health history or insurance coverage. All completed forms may be faxed to the church office at (661) 945-0862. In addition, forms may be turned in during church office hours (8:00 a.m. - 5:00 p.m. Monday through Friday) or at any High School service. Click the link below to download the form.
